Message From the President

Margaret Bean, CNM: Creating a Legacy

Dear midwives and midwifery supporters,

It is my great privilege to share a shining ray of good news with you during these challenging times. The California Nurse-Midwives Association has been gifted with a legacy donation by Margaret Bean, CNM.

Ms. Bean is a retired midwife whose work has spanned continents and countries, always with the focus of centering those on the margins. She has been a staunch advocate for midwives and for the people we serve. I was thrilled last year when Ms. Bean reached out with an offer to fund mentorship and advocacy training for BIPOC midwives in California. She shared her desire to use her funds toward reproductive justice, or as she put it, “birth work reparations.” Her aim is to support the growth of an empowered midwifery workforce who have the skills to shape and advance legislation that serves the people who need us most by eliminating barriers to midwifery. Importantly, her gift is in alignment with our anti-racism platform in that it will prioritize mentees from underrepresented communities.

Over these last few months, getting to know her has been a blessing in and of itself. I’ve enjoyed listening as this wise, elder midwife has shared her stories with me. She’s brought color and depth to my understanding of midwifery in places beyond our borders. And even more, she’s helped brighten our shared vision of what midwives can do here in California.

On behalf of CNMA, it is my honor to say thank you, Ms. Bean. Your legacy will be a healthier California with a diverse and empowered midwifery workforce.

If you or someone you know would like to donate to CNMA’s important work, please make a tax-deductible donation here.

With so much gratitude,

Paris Maloof-Bury, CNM, IBCLC

Immediate Past President

State Policy Updates

Transforming Maternal Health (TMaH) Model

California was one of the fifteen states chosen by the Centers for Medicare & Medicaid Services (CMS) to implement its Transforming Maternal Health (TMaH) Model (TMaH), a 10-year Medicaid and Children’s Health Insurance Program (CHIP) delivery and payment model designed to improve maternal health outcomes, reduce costs, and address serious gaps in health care across California. DHCS will implement the TMaH Model in five Central Valley counties (Fresno, Kern, Kings, Madera, and Tulare).

Federal Policy Updates

Trump Administration Disrupts Provision of Sexual and Reproductive Health Care

The Centers for Disease Control & Prevention (CDC)’s website is being modified to comply with President Trump’s Executive Orders, including the duplicitously named “Defending Women” EO. On Jan 31st it had removed both the  Medical Eligibility Criteria (MEC) for Contraceptive Use and Their 2021 STI treatment Guidelines Guidelines. As of this writing (2/1/25) the STI treatment guidelines were back on the site and the banner read “CDC’s website is being modified to comply with President Trump’s Executive Orders.”

Other pages that were down as of this writing include1:

In the event that they are not available on the CDC site, you can access MEC and STI treatment guidelines along with other sexual and reproductive health guidelines on the Pegasus Health Justice website

Internet pages that have been taken down can often be accessed through the Internet Archive Wayback Machine Website

Trump Administration Halts Freezes Federal Funding Disrupting the Provision of Health Care and other Services

On Monday January 27, the Trump administration issued a sweeping freeze in spending across all  federal agencies. By Wednesday January 28th, the Whitehouse had rescinded the freeze and a federal judge had granted a restraining order to stop the freeze1. The freeze resulted in Medicaid portals (Medi-Cal in California) going down temporarily in all 50 states2. The freeze included all communications, posts, travel, regulation guidance, press releases, social media posts, purchases, funding disbursements, and grant awards that fall under the Department of Health & Human Services, which caused many federal health programs and projects to come to a significant halt. Despite the current rescinding of the freeze it is very unclear what the future holds for federal spending This means the above TMaH funding is in limbo. CNMA will continue to follow this closely. This halt to the functioning of government programs has also halted health care clinic operations in some places.

Addressing Maternal Mortality and ACEs in Reproductive Health

Improving reproductive and maternal health outcomes can have a positive impact on families and can mitigate ACEs and toxic stress later in life. California Surgeon General Dr. Diana Ramos recently launched Strong Start & Beyond, a movement to reduce California’s maternal mortality rate 50% by December 2026. Strong Start & Beyond is focused on fostering awareness, encouraging active participation in health, and building access to resources for mothers, pregnant people, and their families.

Last year, ACEs Aware worked in partnership with Dr. Ramos to host a webinar series called ACEs and Trauma-Informed Care in Reproductive Health. All sessions in the series are available to view on demand and offer continuing education credit.

ACEs Aware continues to present on the topic of ACEs and reproductive health. In September, Sara Johnson, MD, FACOG, an adviser to ACEs Aware, presented at the UCSF Obstetrics, Gynecology & Reproductive Sciences Grand Rounds on addressing ACEs during prenatal care and how providers can build trust and improve health.

Why midwives stay and why midwives leave: Sustaining the midwifery workforce

Almost a quarter of AMCB-certified midwives report not working in the discipline of midwifery. While burnout gets much attention in the media, leaving (and staying in) midwifery is complex and often painful. This session will explore why midwives leave practice and offer individual, organizational, and societal-level strategies to retain midwives based on ten years of research about midwives' professional well-being. Presented by Elisabeth Brie Thumm, PhD, CNM, RN, MBA, FACNM

Do you care for pregnant patients who have undergone a prior cesarean?

We invite you to participate in an interview about barriers to providing equitable care to pregnant people who have had a prior cesarean. We are interested to talk to you in order to better understand the challenges and successes you have when providing care to patients who desire a VBAC or a repeat cesarean.

The study researchers are co-principal investigators Dr. Nicholas Rubashkin from the UCSF Department of Obstetrics, Gynecology, and Reproductive Sciences; Dr. Patience Afulani, in the Department of Epidemiology and Biostatistics; Dr. Margaret Handley in the Departments of Epidemiology and Biostatistics and the Center for Vulnerable Populations; Dr. Miriam Kuppermann from the UCSF Department of Obstetrics, Gynecology, and Reproductive Sciences.

You may be eligible if you: Are a nurse, midwife, physician, or hospital administrator who is involved with providing care to pregnant people who have undergone a prior cesarean.

Are there benefits to taking part in the study? There will be no direct benefit to you from participating in this study. However, the information that you provide may help health professionals better support patients in their pregnancy decisions and eventually may help lower the cesarean rate, which has been identified as a public health priority in California.

How much time does the study take? The first interview will be 30-90 minutes. A second follow up interview lasting 30-60 minutes may be necessary if the researchers have more questions about your story. You may speak with the researcher for up to 2.5 hours over the course of one year.

Where will the interview take place? Over the phone, via the internet, or face-to-face in an agreed upon private place.

Will I be paid? Are there any costs to me? You will receive a $50 gift card for the first interview. You will receive a $25 gift card for a follow-up interview.

Interested volunteers: Send an email to: Nicholas.rubashkin@ucsf.edu

CNMA is looking for submissions for our new social media series “Midwife Crush Monday” where we highlight the stories of trailblazing midwives across the industry who are making a difference in the midwifery profession. You can even submit yourself!

Here’s what we’re looking for:

  • Midwife’s name (include pronouns)

  • Affiliated organization(s)

  • Why you became a midwife (and anything awesome you’d like to brag about)

  • Headshot/Photo if available

  • Bonus: Please include social media handles for tagging!

Please send submissions to sm@cnma.org for consideration!
